Sermorelin (GHRH 1-29 NH2) is a synthetic version of the first 29 amino acids of endogenous GHRH — the shortest fragment retaining full biological activity. It was FDA-approved (as Geref) for treatment of growth hormone deficiency in children. Though Geref was later discontinued due to commercial reasons (replaced by recombinant GH), sermorelin has remained widely used in anti-aging and functional medicine as a safer, more physiological alternative to direct growth hormone injection. It is the most well-established GHRH analogue in clinical anti-aging practice.
Sermorelin binds to GHRH receptors on pituitary somatotrophs, stimulating GH synthesis and release in a pulsatile, feedback-regulated pattern. Unlike exogenous GH, sermorelin preserves hypothalamic-pituitary-axis feedback mechanisms — the pituitary continues to respond appropriately to somatostatin suppression. This means sermorelin stimulates GH release only when physiologically appropriate, reducing the risk of supraphysiological IGF-1 elevation seen with direct GH therapy.
